Memorial Hospital Home HealthCare is a hospital based Home Care agency and have been using TouchPointCare for a little over 6 months in an effort to reduce re-hospitalizations and to improve patient outcomes for our heart failure patients.
It is simple to use and more importantly, our outcomes have been great. Thus far, none of our patients being monitored using TouchPointCare have required re-hospitalization.
Recently, an insurance company who had heard about our monitoring service for CHF patients contacted us. The representative made a comment that their company would like to have their heart failure patients referred to us because of our great outcomes.
The phone monitoring is also well received by our patients and their families. They appreciate the phone follow-up between regularly scheduled visits.
We look forward to expanding the use of the system to improve patient outcomes in disease management.
