TouchPointCare has reduced our average nursing visits on patients with cardiopulmonary assessments from 9 visits per certification period to 7 visits and patients with pain assessments from 11 visits to approximately 8 visits. We are continuing to work on reducing visits for other disease specific diagnoses by adding telehealth. 

Our Acute Care Hospitalization rate has remained at an average of 24%. According to HealthCare Excel this is an excellent outcome. 

The system is very user friendly and initially only a registered nurse had been using the system and now a certified nurse aide uses as well as clerical employees. 

The patients have been very receptive and enjoy the feedback that they receive with the phone calls. 

Support has not been a problem - anytime we needed something we received an answer very quickly and we would recommend TouchPointCare for use by other agencies. Thank you for all the support you have provided to our agency.
