TouchPointCare´s Telehealth Solution

TouchPointCare provides an inexpensive and simple-to-use software application designed for simplifying and managing data collection and reporting.   Where the patient is able to self report, our system lets the health care provider gather any information from the patient that they want, including vital signs and clinical data.  Low cost scales, blood pressure cuffs, glucometers and like monitoring devices are available from most drug stores.  A patient simply weighs themself or takes their blood pressure and reports that information over the phone to the care giver.

Our low-cost telehealth solution avoids the high costs of traditional monitoring equipment while providing many of the same benefits.   It allows easy entry for providers to add it to their suite of health care services. With no investment for providers, TouchPointCare's technology makes it possible for them to offer remote monitoring for all of their chronic care patients.

  • Data is collected by a non-clinician calling a patient on the telephone and asking them a series of questions.  The caller can be an employee of the Provider or the calls can be outsourced to a third party.
  • Patient answers are recorded in the system by the caller.
  • Answers that exceed established thresholds trigger alerts for review by staff clinicians.
  • All answers are stored in our database, hosted on the Internet, and are available for reporting and analysis 24/7.
  • The Provider can create and ask any questions they want for any patient population and they can establish a unique contact schedule for each patient.
