For Consumer Directed Health Plans (CDHPs) TouchPointCare offers a cost effective way to proactively interact with consumers to reduce costs.  Our system enables a provider or health plan to easily implement a customized program designed to:

The TouchPointCare system was designed as a low cost data collection and dissemination tool.  Through the customizable Alerts functionality, participant responses that exceed unique thresholds generate e-mail Alerts to the insurer.  This "management by exception" lets the health plan gather data from as large an at-risk population as desired.  Skilled clinicians need only respond to those participants whose responses triggered an Alert.  The TPC system also makes the data collection as flexible as possible.  Health plans, employers, employees and physicians can collaborate on which of several ways to best communicate...from the low tech telephone to high speed Internet.
