Last year I embarked on development of a case management program for our patients with Chronic Obstructive Pulmonary Disease.
The goals of this program are to improve patient outcomes and decrease frequency and duration of hospitalization. We decided to go with TouchPointCare for several different reasons. One was for their cost effectiveness. One was because of their flexibility; the ease of their software program, and their excellent customer service and support.
TouchPointCare allows us to monitor such data as patient’s weight, their medications, oxygen utilization; patient’s shortness of breath, their mucus production and color, and their hospitalization.
Our results today have been quite significant. We have been able to improve a patient’s quality of life. We’ve been able to decrease their shortness of breath, but most significantly we have been able to decrease our re-hospitalization rate by 32%. I owe a lot of our success to our program to TouchPointCare.
