TouchPointCare Overview

TouchPointCare has developed a software based telehealth application that allows a provider to care for, monitor, and educate patients in their place of residence without the use of expensive monitoring equipment.

Our system is designed to collect and document information from participants via the phone or over the Internet. This data is then stored for review, trending and analysis via a web-based portal by the provider and other system users, including Physicians and family members.

We provide a scalable and fully customizable cost effective solution that improves service delivery and empowers clients to actively participate in their care. Our customers are provider organizations such as home health care, Senior care organizations, DME suppliers, Case management firms, physicians, and other organizations interested in augmenting direct personal visits with "virtual visits" to collect information, ensure compliance or provide some type of monitoring and/or education.
